The game's concept is built on a grieving theme, and we meet a variety of people going through the five stages of grief. In the reels' background, there is a black and white alley in a decrepit district. To form a winning combination, land three or more of the same symbol type on adjacent reels beginning at the leftmost corner.

The lowest-paying symbols are 10, J, Q, K, and A. Landing five of the same symbol type pays between 1.1X and 1.5X the bet. The higher-paying symbols have pink, green, purple, orange, or red backgrounds. If you land five of the same symbol type, the payout ranges from 2.5X to 7.5X the stake.  The whisky and pills icons are the game's scatter symbols, which also double as wild symbols. There are two separate bonus modes that you can trigger, depending on how many scatter symbols you land. These are called Headshrinker Spins and Insanity Spins.

If you land three or four scatter symbols, the Headshrinker Spins feature is activated. You will then begin at level 1 and progress to stage 5, with each stage having a character associated with it. When a win occurs, the character associated with the current level progress will fully expand on the reel. If you create wins with characters that have not yet attained their level, the win multiplier will be increased by 1X for each symbol landed of their sort. This multiplier will subsequently be applied to any winnings generated. You level up by collecting 5 scatter symbols, which grants you an additional 2 free spins.
